Dermal fillers are a fantastic way to enhance facial contours, restore lost volume, and achieve a more youthful appearance. However, like any cosmetic procedure, there are risks involved—one of the most serious being vascular occlusion (VO).
As a qualified aesthetic practitioner, I believe in full transparency and client education. In this blog, I’ll explain what a vascular occlusion is, why it can happen to anyone (regardless of the injector’s experience), how to recognize the signs, and what steps I take to ensure your safety.
Can a Vascular Occlusion Happen to Anyone?
Yes, it absolutely can. A vascular occlusion can occur in any client receiving dermal fillers, no matter how skilled or experienced the injector is. This is because:
✔️ Every person’s facial anatomy is unique.
✔️ Blood vessels vary in location, size, and depth from person to person.
✔️ Even the best injectors cannot see exactly where each vessel lies beneath the skin.
While proper training and advanced techniques significantly reduce the risk, they can never eliminate it completely. That’s why it’s so important to choose an injector who is knowledgeable in vascular complications and emergency protocols.
Vascular Compromise vs. Vascular Occlusion – What’s the Difference?
Vascular Compromise occurs when blood flow is partially restricted, often due to swelling or external pressure from filler. This is usually temporary and can be resolved quickly.
Vascular Occlusion (VO) happens when a blood vessel is completely blocked, preventing oxygen from reaching the skin. If untreated, this can lead to skin necrosis (tissue death).
How to Recognize a Vascular Occlusion
Catching a VO early is key to preventing complications. Here are the main warning signs:
✅ Severe, persistent pain (beyond normal injection discomfort)
✅ Paleness or blanching of the skin immediately after filler is injected
✅ Delayed capillary refill (press the skin—if it stays white for too long, it’s a concern)
✅ Mottled, dusky, or purplish discoloration developing over time
✅ Skin breakdown (if left untreated)
If you notice any of these symptoms after treatment, contact me immediately.
How I Treat Vascular Occlusions
I am fully trained in vascular occlusion management and take immediate action to restore blood flow if an issue arises. My emergency protocol includes:
✔️ Stopping the procedure immediately
✔️ Massaging the area (in specific cases)
✔️ Applying warm compresses to encourage circulation
✔️ Administering hyaluronidase (if using hyaluronic acid filler) to dissolve the blockage
✔️ Encouraging blood flow with aspirin or vasodilators (if necessary)
✔️ Providing ongoing monitoring and aftercare to ensure full recovery
Why It’s Important to Choose a Trained Injector
Although vascular occlusions are rare, they can happen to anyone. It doesn’t matter if your injector is a medic or non-medic—the risk is the same. What does matter is whether they have the knowledge, training, and ability to act fast in the event of a complication.
As a solo practitioner, I take client safety extremely seriously. I have extensive training in facial anatomy, safe injection techniques, and emergency protocols, ensuring that you are in the safest possible hands.
